Case Manager/Care Coordination- Tallahassee, FL
Help youth and families build brighter futures! In this role, you’ll provide professional case management services, connect families to resources, advocate for their needs, and guide them through various systems of care.
Key Responsibilities:
Support safety, permanency, and well-being
Develop and monitor service plans
Build strong relationships with families and community partners
Provide crisis intervention when needed
Requirements:
Bachelor’s degree in Human Services or related field
1+ year experience with children and families
Valid driver’s license and reliable transportation
