The Maryland Developmental Disabilities Council, in partnership with the Parents' Place of Maryland, has developed this survey specifically looking at the transition of students with disabilities from school. The August 3rd deadline is now extended to August 17th.
Transition planning is a process mandated by the Individuals with Disabilities Education Act (IDEA 2004) for all students who have an Individualized Education Program (IEP). The purpose is to facilitate the student's move from school to post-school activities, including college, work, and other community activities. In Maryland, this transition planning begins at age 14.
We want to learn from families about their experiences, concerns, and ideas about what works and doesn't work. The information provided will be used by our organizations to expand our understanding of families' experiences, identify potential gaps and issues that exist, identify successful practices that could potentially be replicated, and guide our work.
Please take about 15-20 minutes to complete the survey by Friday, August 17th.
