On average, you should be washing and conditioning your hair at least once a week. However, there can be a few things that will determine whether you should wash and condition your hair more often. Examples few things to look at include: what you're eating, what medications you're taking, what activities you're participating in, i.e. exercise, working outside, etc.
All these things determine how often you should be washing and conditioning your hair. They all can play a factor on how healthy your hair is and how your hair will look and feel. If you exercise, this will involve you washing and conditioning your hair more often. For example, if you workout four times a week, you should wash and condition your hair after your workout.
