Patient and Family Navigator - Pediatric Intellectual and Developmental Disability
Patient and Family Navigator who coordinates care for pediatric and young adult patients and their familiesespecially those with autism, intellectual and other developmental disabilities (IDD)by supporting patients and families as they navigate UVA Medical Center and community resources. Provides ambulatory care coordination in collaboration with clinician recommendations, including frequent communication, written documentation, registry maintenance, interpreter collaboration, and care follow-up.
Requires 3+ years of care coordination experience, strong problem-solving skills, and the ability to work both independently and within an interdisciplinary team. Role includes in-clinic and virtual care and occasional evening hours.
Individual contributors with responsibility in a professional discipline or specialty. Spends majority of time leading the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills normally acquired through advanced education (typically University).
Established and productive individual contributor. Works independently with general to limited supervision. Problems faced are difficult and may be complex. May influence others regarding policies, practices, and procedures. May have project management responsibilities on small, less-complex projects.
MINIMUM REQUIREMENTS
Education: Bachelors degree required. Equivalent combination of education and experience may be considered in lieu of degree.
Experience: 3 years relevant experience.

PHYSICAL DEMANDS
This is primarily a sedentary job involving extensive use of desktop computers. Depending on department, this job may occasionally require traveling some distance to attend meetings, and programs.
The starting base rate for this role is $47,512.00 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
