Job Description

The Patient Coordinator is responsible for engaging with patients due for cancer screening, identifying barriers to cancer screening, facilitating the organization of phlebotomy, and answering questions related to Guardant Cancer Screening tests. The coordinator will provide exceptional support to Guardant Health Screening and Patient Navigation. The coordinator must possess the ability to communicate professionally and effectively with all individuals, including external customers and internal Guardant Health personnel.

 

Essential Duties and Responsibilities:

 ·       Receive and respond to telephone, email as well as web inquiries.

·       Document all triage and follow up activities.

·       Monitors patient information and result tracking in Salesforce.com.

·       Adheres to Standard Operating Procedures (SOP’s) pertaining to Patient Navigation requirements.

·       Performs quality checks to ensure accuracy of data entry.

·       Works with the screening team to ensure client information is correct.

·       Manage and distribute materials as requested.

 

Position Requirements:

 

·       Experience required with healthcare, medical terminology, and interacting with patients

·       High school diploma, or its equivalent required with healthcare experience. Associate or bachelor's degree preferred.

·       Strong ability towards teamwork that is comprised of patients, colleagues, contractors, and leaders.

·       Ability to speak Spanish preferred

·       Flexibility to multitask and take on various projects.

·       The ability to work with self-discipline, good judgment, and independence in a dynamic office setting.

·       Excellent written and verbal communication skills are required, as is computer proficiency.

·       Solid experience using  Microsoft Office (Word, Excel).

·       Full Time position, with flexibility with respect to working hours based on operational need.

·       Patient Coordinator is will be located within at the Guardant Health office in Spring, Texas.

 

Work Environment:

 The employee may be required to routinely lift office supplies and use office equipment. Majority of the work is performed at a desk in an office environment: however, there is a potential exposure to high noise levels, fumes, and biohazard material in a laboratory environment. The employee must have the ability to work at a computer for extended periods of time.

 

Delegated Duties and Responsibilities:

 

All duties as assigned or delegated from the Director of Patient Navigation.
